One could take this dud thing one farther: dud equipment. How about engine failure? Turbocharger failure at altitude? Hydraulic failure - one gear won't retract. The list is endless. Yes, random is hard to do but maybe it wouldn't have to be random. A pattern that repeats itself over a long period of time is hard to detect. Say, an engine failure every 75 minutes of accumulated flight time using a log of some sort. This log could be user-configurable to extend/shorten the time or reset it altogether.
