I get the same problem sometimes too. Cause is unknown - apparently it is a known bug and not related to MODS (according to M4T folk who play sans MODs). It doesn't just happen autogen campaigns - but also linear ones too. Only fix I know is to change the flights so that your squad is the only one in that particular flight.
