I think the pressure inside the boiler is too high and something who doesn't penetrate but modifies the boiler's structure already causes an explosion because the high pressure inside it.
Five men died near my work place, the cause was a wrong control of pressure of a industrial boiler resulting in an explosion.
My 2 cents.
