How can I run Il-2 Mat Manager for two seperate installs of Il-2 ?
Do I have to run the setup exe twice or can I use the single MatManager install for both?

I had this issue for awhile, too.

You only get one at a time, and you have to point to the root directory of the install you want to use it on (Use Setup in MAT Mannager). Don't forget to rename the .exe files as instructed elsewhere. You probably need the files in the install you want to use it on, too.
