I have found the stock 4.09 patch reduced my fps by 20-30fps from the stock 4.08. I experimented with all the dll's available for download. But when I used the Unified Installer the 4.09 mod package it is the best for my system. I'm back up to 60fps maxed out with vsync on. I did however notice some settings in your nvidia settings that are not optimized.
Negative LOD bias Some games force a negative LOD BIAS nevertheless. The result is heavy texture shimmering. To avoid this, the driver can clamp the LOD BIAS to zero. That means that the LOD BIAS can still be raised above zero, but it cannot set lower than zero.
Frames to render ahead: Using this setting, you can limit the amount of frames the driver accepts ahead of the currently displayed frame. This should be left at the default value (3) to get the best performance.
You can get a faster reaction on your mouse movements in some games by lowering the limit. This might also cause a loss in framerate, though.
Threaded Optimization Threaded Optimization: Controls the use of multithreaded optimization for all 3D games on Dual Core/HyperThreading systems. The available settings are Auto, On and Off. I would strongly recommend the default option of Auto, allowing the drivers to set this appropriately for various games based on your hardware. Only turn Off for troubleshooting purposes if you believe a particular (older) game is not compatible with dual core CPUs.
Triple Buffering If set to On, this setting allows your overall performance to improve when Vertical Synchronization (VSync) is enabled in games. Therefore if you want to enable VSync - whether in the in-game settings or in the Nvidia Control Panel, it is recommended you enable Triple Buffering as well. However note that using Triple Buffering may cause problems for graphics cards with lower Video RAM, so disable this option if you
