Check your config.ini and make sure that the screen resolution your trying to run the game in is within the limits of your video card and that it's got everything in it.
Also make sure that Vista has been patched to SP1.

If your config.ini file is or looks messed up. Even if you're just not positive, delete your config.ini file and re-run the IL2setup.exe file. This will re-build the config.ini file back to baseline.
