Leave VSync ON for TrackIR or you will have vertical tearing as you turn your head.

If you do not have TrackIr, or I assume any other kind of head tracking device, you may be able to leave it off. Your FPS will be higher, but if you are using any viewing device, including mouse look, you may get tearing if the game can't draw the image fast enough to keep up with it.
